What is Scalp Micropigmentation?

Scalp Micropigmentation (SMP) — sometimes called a "hair tattoo" — is a non-surgical cosmetic procedure that uses a specialised micro-needling device to deposit medical-grade pigment into the upper dermis of the scalp. The technique replicates the appearance of hair follicle dots — creating the visual impression of a close-cropped, freshly shaved head, or adding the appearance of density and coverage to thinning hair.

Unlike traditional tattooing, SMP uses specialised organic pigments (formulated specifically for scalp application), finer needles, and a much shallower penetration depth to create a natural-looking result that mimics the appearance of real hair stubble. When performed by a skilled practitioner, SMP is virtually undetectable as a cosmetic procedure.

SMP is not a replacement for hair transplant — it does not grow real hair. Rather, it is a cosmetic camouflage solution that creates the visual impression of hair where physical hair is absent. It is particularly valuable for patients who are not candidates for hair transplant, who prefer a shaved aesthetic, or who wish to camouflage visible scars.

What SMP Can Achieve

  • Shaved head appearance (for extensive baldness): SMP creates the realistic appearance of a fully shaved head — with thousands of individually placed pigment dots replicating the look of a zero-grade haircut. Ideal for Norwood V–VII patients who prefer this aesthetic over transplant, or who do not have sufficient donor supply for comprehensive transplant coverage.
  • Density illusion (for thinning hair): For patients with diffuse thinning (rather than complete baldness), SMP pigment dots placed between existing hair create the visual impression of greater density — reducing scalp visibility and making thinning far less apparent.
  • FUT scar camouflage: The linear scar left by FUT (strip) surgery can be camouflaged effectively with SMP — pigment dots within and around the scar break up its linear appearance and make it blend with the surrounding donor hair at any length.
  • FUE dot scar camouflage: While FUE leaves much less visible scarring than FUT, patients with very short hair and higher-density extraction scars can benefit from SMP to reduce their visibility.
  • Hairline definition: SMP can add definition to a receding hairline without surgery — creating a sharper, more defined frontal hairline using pigment. This is a temporary and non-permanent solution compared to hair transplant.

The SMP Procedure

  1. Design consultation: The desired hairline shape, extent of coverage, and pigment shade are discussed and mapped. Pigment colour is matched to your hair and skin tone for a natural result.
  2. Session 1: A lighter base layer of pigment is applied across the treatment area — establishing the foundation of the design at a lighter shade. 2–3 hours duration. Mild redness for 24 hours.
  3. Session 2 (7–14 days later): Additional layering darkens and densifies the result, adding definition and realism. The hairline is refined and additional dots fill any gaps.
  4. Session 3 (optional, 7–14 days later): Final refinement — perfecting density, shade, and hairline naturalness. Ensures optimal longevity of the result.
  5. Aftercare: Avoid sun exposure, swimming, and sweating for 5–7 days after each session. Moisturise the scalp daily to maintain pigment vibrancy. Use SPF 30+ on the scalp when outdoors (UV exposure is the main cause of premature fading).

SMP Longevity & Maintenance

SMP pigment gradually fades over time as the body naturally metabolises foreign particles in the skin and UV exposure breaks down pigment molecules. Typical longevity:

  • Most patients need a touch-up session every 3–5 years to refresh the result
  • Longevity is significantly improved by: regular scalp SPF use, avoiding prolonged sun exposure, and proper aftercare
  • Fading is gradual and natural-looking — it does not change colour or turn blue/green (a risk with poor-quality pigments)
  • Touch-up sessions are quick (1–2 hours) and significantly less expensive than the initial procedure

Frequently Asked Questions

Will SMP look natural up close?
+
When performed by a skilled practitioner using quality pigments and proper technique, SMP is highly convincing up close. Individual dots are placed at irregular spacing (just like natural follicle distribution) and at varied angles. The result mimics the appearance of a 1–2 day stubble so convincingly that most people cannot distinguish it from real hair stubble, even on close inspection.
Can I have SMP and hair transplant?
+
Yes — SMP and hair transplant are highly complementary. For patients with Norwood V–VI, transplant can restore the frontal hairline and mid-scalp, while SMP covers the crown area where donor supply may be insufficient for full transplant coverage. The two treatments together can create a very comprehensive, natural-looking result.
Does SMP work for women?
+
Yes — for women with female pattern hair loss (especially Ludwig Stage II–III), SMP can be used to reduce the visual contrast between sparse hair and visible scalp. Pigment dots in the parting and crown area create the impression of denser hair. For women who retain their hairline, SMP is a powerful density-adding tool.
